To finalize your application, please ensure to complete all the tasks outlined in the checklist below, which includes submitting all supplementary application forms to your FACTS Application.

Application Checklist & Portal

for Incoming Freshmen


For students coming from our Foundation schools: St. Joseph's, St. John Neumann, Sacred Heart, St. Mary's or Episcopal School of Knoxville:

  • Submit completed online application and application fee of $125.
  • Register for the High School Placement Test (The next placement tests will be on December 6, 2025 at 8 a.m.) The KCHS Access Code is ZN.
  • Submit signed Request for Records form to your current school through the FACTS application system.
  • Submit completed Personal Statement Form to the FACTS application portal.

For students not coming from a Foundation school:

  • Submit completed online application and application fee of $125.
  •  Register for the High School Placement Test (The next placement tests will be on December 6, 2025 at 8 a.m.) The KCHS Access Code is ZN. 
  • Submit signed Request for Records form to your current school through the FACTS application system.
  • Submit completed Personal Statement Form to the FACTS application portal.
  • Access the Letter of Recommendation in the FACTS application portal and enter the email of the individual completing a personal reference on the student's behalf.
  • Have your current 8th grade teacher(s) complete the 8th Grade Student Evaluation Form on your behalf by entering their email to the FACTS application portal.

For Transfer Students:

  • Submit completed online application and application fee of $200.

  • Submit signed Request for Records form to your current school through the FACTS application system.
  • Submit completed Personal Statement Form to the FACTS application portal.
  • Access the Letter of Recommendation in the FACTS application portal and enter the email of the individual completing a personal reference on the student's behalf.
